Elmwood Early Learning Center Enrollment Form

Below you will find all required forms needed prior to your child's start date. Child health report MUST be signed by your physician. Word documents can be filled in, PDF forms must be printed and filled out.

Early Learning Center

Nestled within the Elmwood School District building, the Elmwood ELC opened its doors on September 1, 2017. The ELC can be found at the southern end of the elementary wing, with its own secured entrance. The ELC is split into two rooms, the Mighty Wolves Room (ages 2-4) and the Little Wolves Room (3 months-1 year). The children enrolled in our program are offered a variety of experiences which include but are not limited to specific daily schedules that are age appropriate per room, story time at the school and public libraries, walks around town, outside play time, free time in the school's gymnasiums, and much more!
